Rehab HQ Exercise Vault Liability Waiver and Release
PLEASE READ CAREFULLY BEFORE USING THIS PROGRAM
This Liability Waiver and Release (“Agreement”) is entered into between the participant (“Member”) and Rehab HQ, LLC (“Company”), including its owners, employees, contractors, and affiliates. By accessing or participating in any exercise program, video, or related content (collectively, the “Program”), Member acknowledges and agrees to the following terms:
1. Voluntary Participation and Assumption of Risk
Member understands that the Program allows self-selection of exercises and that participation is entirely voluntary.
Member acknowledges that:
- Exercise involves inherent risks of injury, including but not limited to muscle strain, joint injury, falls, loss of balance, and other serious conditions.
- It is Member’s responsibility to consult with a physician or licensed medical provider before starting any exercise or rehabilitation activity.
- By choosing exercises independently, Member assumes full responsibility for determining what is safe and appropriate for their individual condition.
Member voluntarily assumes all risks, known and unknown, associated with participation in the Program.
2. No Medical Advice or Supervision
Member understands that the Program provides general educational and informational content only and is not a substitute for personalized medical advice, diagnosis, or treatment.
No part of the Program creates a therapist–patient relationship. All exercises should be performed at your own pace and within your own limits.
3. Release and Waiver of Liability
To the fullest extent permitted by law, Member hereby waives, releases, and discharges Rehab HQ, LLC, its owners, affiliates, employees, and contractors from any and all claims, damages, injuries, or losses arising out of or in connection with participation in the Program.
This includes, without limitation, any claims of negligence, breach of duty, or product liability.
Member further agrees to indemnify and hold harmless Rehab HQ, LLC from any claims brought by third parties arising from Member’s participation in the Program.
4. No Guarantee of Results
Member acknowledges that individual outcomes vary and that Rehab HQ makes no guarantees of recovery, improvement, or specific results.
5. Acknowledgment and Acceptance
By completing checkout, creating an account, or accessing the Exercise Vault, Member affirms that they have:
- Read and understood this Agreement;
- Had the opportunity to ask questions;
- Voluntarily agreed to all terms.
By checking the agreement box during checkout, Member provides a legally binding electronic acknowledgment in place of a physical signature.
